An interdisciplinary and interprofessional coalition dedicated to delivering value-informed, compassionate and appropriate medical care to senior patients

We provide:

  • Comprehensive education for health professionals and learners in geriatrics, dementia care, palliative medicine and advance care planning.
  • Community engagement, outreach and education for aging patients and their caregivers.
  • Project development and data-sharing to promote quality improvement and the creation of an “age-friendly” health system.
  • Public health messaging across platforms to promote compassionate care for senior patients.

Education

The What Matters Most Virginia curriculum focuses on developing the necessary skills to be effective caregivers and stewards for their patients.

We're committed to preparing the next generation of family doctors to meet the challenges of the future. 

Traditional lectures combined with simulated and real-life patient experiences expose our learners to the real-world challenges faced by aged patients and those living with complex diseases.

Senior Strong

Family & Community Medicine’s Senior Strong clinic provides family-centered care aimed at maximizing quality of life for seniors navigating the challenges of aging and/or medical illness.

By focusing on what matters most to patients and their loved ones, our interdisciplinary team strives to align medical care with our patients’ values in order to deliver highly personalized and patient-centered care.  In addition to physicians, our team includes specialists in behavioral health, nursing, and care management.  

Our services include:

  • Cognitive Evaluation for Memory
  • Full Evaluation and appropriate treatment for mood challenges such as Depression and Anxiety
  • Comprehensive review of medications and reduction of medications considered high-risk for seniors
  • Coordination of care and enrollment in services such as physical and occupational therapy
  • Advance Care Planning, living wills and durable medical orders

Community Outreach

What Matters Most Virginia engages and provides education to the community through a variety of community events, including interactive education and expert panels.
