If you were exposed to a COVID-19 positive person, a person under ivestigation (PUI) or have developed symptoms that you think might be consistent with COVID-19 please follow the instructions listed below.

You will need to have basic information about the COVID-19 positive person you were exposed to, including the date of the contact/exposure and the date that person was tested (if known). You do need to follow the normal procedure for notifying your supervisor that you will not be reporting for work or, if on campus, that you are leaving immediately because of exposure or symptoms.

Complete and submit the REDCap EVMS Employee Exposure/Symptom Survey, which can be found at this link.

  • When you click on the button to submit the survey, you will see a “disposition” based on your responses. Read it carefully.
  • If the disposition indicates that Occupational Health will contact you, they will do so by noon of the following business day. If they haven’t contacted you by then, call them at 757.446.5870. In case of urgency or concern and only outside of Occupational Health normal business hours (8:30 a.m. to 4:30 p.m.), you may call their exposure pager at 757.669.1157.
  • After talking to Occupational Health, if testing is indicated, call the EVMS Medical Group Quality Office at 757.451.6232 to schedule your drive-through testing. Then, print the applicable self-monitoring form (Symptomatic or Asymptomatic).
  • Start monitoring your symptoms according to the instructions on the form. Failure to follow the instructions on the form will delay your return to work clearance.
  • Test results are provided via phone by the EVMS Medical Group Quality Office. (Please note that Occupational Health does not provide test results.) On average, it takes one to three days to receive test results, but it could take longer. Negative test results are provided during normal business hours Monday through Friday. Positive results are provided as soon as test results become available to the EVMS Medical Group Quality Office.

TEST RESULTS, EVEN IF NEGATIVE, DO NOT AUTHORIZE YOU TO RETURN TO WORK. THE FOLLOWING STEPS MUST BE FOLLOWED IN ORDER TO BE CLEARED TO RETURN TO WORK.

  • When you receive your test results, contact Occupational Health by phone at 757.446.5870 or email at occhealth@evms.edu. They will provide you with further instructions regarding your return to work clearance.
  • At the appropriate time, based on the instructions in the self-monitoring form or your communication with Occupational Health, you will need to send your self-monitoring form to Occupational Health. Occupational Health will then review your test results and self-monitoring form to ensure that you have met the return to work clearance criteria. They will then issue you a return to work clearance notification, if indicated.
  • If you are not cleared to return to work, Occupational Health will inform you of the required next steps to be taken before you can be cleared. YOU MAY NOT REPORT TO WORK UNTIL CLEARED BY OCCUPATIONAL HEALTH.
  • EVMS/EVMS Medical Group faculty, staff, residents and fellows should not be scheduled for duty until the employee has received a return to work clearance notification from Occupational Health.
