EVMS has now received an allocation of the Pfizer vaccine and has begun scheduling appointments in accordance with the EVMS priority list for EVMS faculty, staff and students interested in receiving the vaccine. Clinics began today and you will receive an email with scheduling instructions when you are eligible to receive the vaccine. You must schedule an appointment to receive the vaccine; no walk-ins will be accepted.

By scheduling the first appointment, you are committing to receiving both doses of the COVID-19 vaccine. You will be required to show up for your appointment on time and return for a follow up appointment 21 days later. The vaccine is in high demand and we must do our part to avoid any waste, including partial vaccination.

Before scheduling your appointment, please review the following materials and talk to your healthcare provider about any specific questions:

  1. Pfizer Vaccine Fact Sheet. You will be provided a copy of the Fact Sheet at the time of vaccine. If you have any questions about the vaccine or whether it is right for you, please speak with your healthcare provider.
  2. CDC Pre-vaccination Checklist. Please review the checklist and discuss any “yes” answers with your healthcare provider. If you answer “yes” to questions #1, 3 or 6, you will not be able to get the vaccine at this time. These conditions include currently being sick, having a vaccine within 14 days, or previously having a severe allergic reaction to the vaccine or an ingredient in the vaccine.
  3. EVMS Consent and Waiver Form. You will be required to complete and sign this form at the vaccine clinic. It includes the CDC Pre-vaccination checklist and an EVMS Consent and Waiver form.
  4. EVMS Vaccine Distribution FAQs are available on the web and MyPortal, or by navigating to Preparing for Your Vaccination at EVMS on MyPortal.

We remind everyone that, even after vaccinations are widespread, it is absolutely essential that our masking, social distancing and sanitation protocols continue to be followed. The risk of transmission of the COVID virus will be mitigated, but will remain for months to come.
